I have tried to reproduce this issue but failed. It could be a has a chance of theme or plugin conflicts. You can disable all your plugins except User Frontend and switch to a default theme of WordPress like- Twenty Twelve, Twenty Thirteen, Twenty Fourteen and let me know the results.

Step 1: Please navigate to plugins/wp-user-frontend/class/render-form.php

Step 2: Find
$username = sanitize_user( substr( $email, 0, strpos( $email, '@' ) ) );

Step 3: Replace the following line over the previous line.
$username = $email;

Just have opened Plugin 2.4.5/assets/css/ folder and renamed style.css file to style_backup.css. Then I have uploaded the new style.css file (which I have told you to download from the link) to this folder `Plugin 2.4.5/assets/css/. Now everything is working.
